On May 1st, 2006 my middle name was legally changed from 'John'  to 'Joanna'.
   The order was signed by a Judge of the 11th District Court of Crawford County
   Kansas and was duly registered with the Crawford County clerk on the same date.

   At some point in time, after my facial surgery is completed, I intend to change my
   first name to 'Joanna' and use my current first name as my middle name.

   But whatever else may happen, for now and forever, I am truly joanna.  Nothing
   can ever change that.
